This policy explains what personal information this site collects, why, and what I do with it.

Who is responsible

Julie Gazard Ac., acupuncturist, member of the Ordre des acupuncteurs du Québec, operates this site. I see clients at the Action Sport Physio Maisonneuve-Rosemont clinic, 2815, rue Sherbrooke Est, local 200, Montréal (Québec) H2K 1H2.

What is collected

The contact form collects what you enter in it yourself:

your name
your email address
your phone number, if you choose to enter it
the message you write

Nothing else is collected without your knowledge. This site uses no advertising tracker and does no profiling. Online booking happens on the platforms of the clinic and of the SLAB, which have their own policies.

Why, and for how long

This information is used only to reply to you. It is never sold, rented or traded.

It is kept for as long as it takes to handle your request. If a file is opened at the clinic, the record-keeping rules of the Ordre des acupuncteurs du Québec apply to that file, not this site.

Cookies

This site sets a single cookie, the one that remembers your choice about the cookie banner, so that you are not asked again on every visit.

No audience measurement, advertising or social media cookie is set. The links to Facebook and YouTube are simple links: nothing loads until you click.

Your rights

You can request access to the information about you, its correction, or its deletion where the law allows.

A request is made in writing to juliegazard.ac@gmail.com. I reply within thirty days.

You can also file a complaint with the Commission d'accès à l'information du Québec.

Security

Exchanges with this site are encrypted. Messages received through the form arrive in my email inbox, to which access is protected.
